Telehealth/Video Consultation – Disclaimer

I understand that my health care provider (Royal Bahrain Hospital) is providing me with a Video Consultation. This means that I and/or my healthcare provider or designee will, through interactive audio/video connection, will be able to consult with the clinician (assigned by Royal Bahrain Hospital) about my condition.

I understand there are potential risks with this technology:

1. The video connection may not work or that it may stop working during the consultation.

2. The video picture or information transmitted may not be clear enough to be useful for the consultation.

3. I may be required to go to the location of the consulting physician if it is felt that the information obtained via telemedicine was not sufficient to make a diagnosis.

I also understand other individuals may be present to operate the video equipment and that they will take reasonable steps to maintain confidentiality of the information obtained. The video call may also be recorded for record keeping purposes.

I understand that a limited physical examination will take place during the videoconference and that it cannot replace a full medical examination done physically at the medical centre/clinic. I have the right to ask my healthcare provider to discontinue the conference at any time. The video consultation services are not in any way intended to be used in case of an emergency.

I also understand that the amount paid for the consultation is non-refundable.

I authorize the release of any relevant medical information about me to the consulting health care provider, any staff the consulting health care provider supervises, third party payers and other healthcare providers who may need this information for continuing care purposes.

I also understand that the responsibility of the consulting provider regarding my health care will terminate upon conclusion of the teleconference.

I hereby release Royal Bahrain Hospital, its personnel and any other person participating in my care from any and all liability which may arise from the taking and authorized use of such Video/E Consultation, Videotapes, Digital recording films and Photographs. I have read this document and understand the risk of the video consultation.

RBH will be strictly following the patient confidentiality during the telemedicine services

All the patient information will be recorded in the Electronic Medical Records (EMR), and the documents are kept for five years

NHRA has the right to check the documents at any point of time

I hereby consent to participate in a Video consultation under the conditions described in this document.

إخلاء مسؤولية – استشارة رعاية صحية عن بعد / بالفيديو

أتفهّم أن مقدم الرعاية الصحية الخاص بي (مستشفى رويال البحرين) يقدم لي استشارة بالفيديو. وهذا يعني أنني و / أو مقدم الرعاية الصحية أو من ينوب عنه، من خلال التواصل التفاعلي بالصوت / الفيديو، سيتمكنون من التشاور مع الطبيب (الذي تعينه مستشفى رويال البحرين) حول حالتي

:وأقر أن هناك مخاطر محتملة مع هذه التقنية حيث 

 قد لا تعمل تقنية التواصل بالفيديو أو أنه قد يتوقف عن العمل أثناء الاستشارة -  

    قد لا تكون صورة الفيديو أو المعلومات المرسلة واضحة بما يكفي لتكون مفيدة للاستشارة -

قد يُطلب مني الذهاب إلى موقع الطبيب الاستشاري إذا شعرت أن المعلومات التي تم الحصول عليها عن طريق الرعاية الصحية عن بعد لم تكن كافية لإجراء التشخيص -

كما أتفهم أنه يجوز حضور أفراد آخرين لتشغيل معدات الفيديو وأنهم سيتخذون خطوات معقولة للحفاظ على سرية المعلومات التي تم الحصول عليها. وقد يتم تسجيل مكالمة الفيديو أيضًا لأغراض حفظ التسجيل

أتفهم أيضًا أنه سيتم إجراء فحص بدني محدود أثناء الاجتماع عن طريق الفيديو، وأنه لا يمكن أن يحل محل الفحص الطبي الكامل الذي تم إجراؤه جسديًا في المركز / العيادة الطبية. ويحق لي أن أطلب من مقدم الرعاية الصحية الخاص بي عدم مواصلة الاجتماع عن طريق الفيديو في أي وقت. ولا يُقصد من خدمات استشارة الفيديو بأي حال من الأحوال استخدامها في حالة الطوارئ

كما أقر بأنه لا يمكن استرداد المبلغ المدفوع للاستشارة

أفوض بالإفصاح عن أي معلومات طبية ذات صلة بي لمقدم الرعاية الصحية الاستشاري، وأي موظفين يشرف عليهم مقدم الرعاية الصحية الاستشاري، والجهات الدافعة الأخرى ومقدمو الرعاية الصحية الآخرون الذين قد يحتاجون إلى هذه المعلومات لأغراض الرعاية المستمرة

وأتفهم أن مسؤولية مقدم خدمة الاستشارات فيما يتعلق برعايتي الصحية ستنتهي عند نهاية الاجتماع عن طريق الفيديو

وبناء على ذلك أخلى مسؤولية مستشفى رويال البحرين وموظفيه وأي شخص آخر مشارك في رعايتي من أي وجميع المسؤوليات التي قد تنشأ عن أخذ الاستشارة بالفيديو / عن بعد وأشرطة الفيديو وأفلام

التسجيل الرقمي والصور الفوتوغرافية واستخدامها المفوض به. وقد اطلعت على هذه الوثيقة وأدركت المخاطر التي قد تنطوي على تقديم الاستشارة بالفيديو

سوف يتبع مستشفى رويال البحرين خصوصية المريض اثناء الاستشارة بالفيديو

جميع معلومات المريض سوف تحفض في ملف/سجل المريض الطبي لمدة ٥ سنين

الهيئة الوطنية لتنظيم المهن والخدمات الصحية لديها الحق للأطلاع على سجلات المريض في أي وقت 

وعليه أوافق بموجبه على المشاركة في الاستشارة الفيديو وفقًا للشروط الموضحة في هذه الوثيقة
